Marine Inquiry into the MV ANJEANETTE Casualty

The Board commenced work on 1 September 2026 and comprises three members. The Board will examine the circumstances and contributing factors, relevant operational and emergency-response matters, possible failures of duty or legal non-compliance, and safety actions required to prevent similar casualties. Its written report is due by 31 October 2026.

The Board will conduct the Inquiry independently and in accordance with the Shipping Act 1998 and the Shipping (Marine Inquiries and Investigations) Regulations 2011. Information about the public hearings, attendance arrangements and participation in the Inquiry will be provided in the General Notice of Marine Inquiry once approved by the Board.

On 4 September 2026, the Board has published the General Notice of Inquiry and Procedural Direction No.1 for Conduct of Inquiry, outlining the public hearing arrangements, participation requirements and hearing timetable commencing 14 September 2026.

Public members wishing to attend the hearings, please download the Application Form and send to beima.taisia@sima.gov.sb or leave it at SIMA Office reception in Point Cruz by Saturday 12 September at 5pm.

On 18 September, the Procedural Direction No.2 has been issued by the Board of Marine Inquiry to explain the next phase of the Inquiry, including the review of evidence received to date, consideration of supplementary evidence and witnesses, potential supplementary hearings, the procedural fairness process for any person who may be affected by the Board’s findings, and the preparation of the final report. The Board has not yet closed the evidentiary phase of the Inquiry and may continue to receive and assess additional evidence before finalising its findings, conclusions and recommendations.
